If you have an appointment already, we look forward to meeting you.

In order to provide you with the best possible care, we request you print off the following forms and bring the completed forms and other items listed below with you.

• New Patient Form
• A list of all your medications
• Your current health insurance card
• An authorization/referral form if you have a managed care insurance

If you have any past medical records that may be important, please print and sign our Medical Records Release and present this to your doctor requesting those records to be sent to our office prior to your eye exam.
